    Please have your student's doctor complete the appropriate Health Action Plan for Chronic Health Conditions and return it to the clinic as soon as possible so I can notify your student's teachers and be prepared to respond to your student's needs. The forms can be downloaded by clicking HERE
     
    Please return forms to the clinic in person or by FAX (470) 695-7828.  ALL MEDICATION must be brought in to the clinic by a parent or guardian.
     
     

    23-24 School Year Clinic Reminders

     

    I have enjoyed helping to care for the health needs of your children this year! There are many things to think about as we approach the last week of school. Here are just a few reminders from the clinic:

    New Health Forms/Meds for the 2023-24 School Year - With each new school year, it is necessary to update your child’s Health Forms in the school clinic. All students with medical conditions (ie Asthma, Allergies, Seizures, etc.) must submit new health care plans each school year. The forms must be completed, signed by parent and physician, dated after 5/26/23, and turned in to the School Nurse when school resumes in August. Please see the school website: Student Services/School Nurse for more information.

    If you plan to bring medications to the clinic next year, you must complete a Request for Administration of Medication form with each medication turned into the clinic. If the medicine is over the counter, only a parent's signature is required. However, if it is prescription medication, or needs to be administered for more than 2 weeks, the form must also be signed by a physician. Please see the school website: Student Services - School Nurse - Medications - to read all the information regarding safely transporting and keeping meds in the clinic.

    If you would like for your student to carry emergency medication, such as an inhaler, epi-pen, or glucagon/diabetes supplies, the physician must indicate on both the Care Plan and the Authorization for Students to Carry form that the student is trained and capable of carrying and self-administering the listed medication. The Authorization for Students to Carry form requires physician, student, and parent signatures.

    If you have a rising 7th grader, please contact your student’s primary care provider to see if they will need any required immunizations before school resumes in August.
