• Health Forms

     

    New Health Care Plans are required at the beginning of every school year. Please click here to view and print the health forms needed for your students. We also must have a Request for Administration of Medication form for every medication that is kept in the clinic. Please note that many of the forms do require a physician's signature. Forms may be returned in person, via email to kjackson@forsyth.k12.ga.us, or via fax to 770-667-2593.
