Attention Rising 7^th Grade Students:

Dear Parent/Guardian, 

According to Georgia State Law, children who are entering 7th grade must have received one (1) dose of Tdap (Tetanus, Diphtheria, Pertussis) vaccine and one (1) dose of MCV4 (meningococcal conjugate vaccine). 

ºìÐÓÖ±²¥app must receive an updated immunization form (GA 3231) for your child before the 2024-25 school year starts.  

If you wish to seek an exemption, you must provide one of the following:  

·       GA Form 2208 (Affidavit of Religious Objection to Immunization)  

·       Medical exemption noted on GA Form 3231 by the physician.  

Please contact your child’s healthcare provider or the Forsyth County Health Department located at 428 Canton Rd, Cumming, GA  30040 (770-781-6900) at your earliest convenience. If you need any assistance or have any questions, please contact your child’s school nurse. Once your child has received their Tdap and MCV4 immunizations, please provide a copy of their updated GA 3231 form to your child’s school nurse.
