    A note from Nurse Cantrell & Nurse Poppe:

    -In the event the Clinic needs to reach you during the school day, please make sure that all contact phone numbers are updated in your Parent Portal account (under Demographics).  Consider adding the school's telephone number to your phone's contact list so that "West Forsyth High School" will show up on your caller ID. Here is a guidance related to viruses/infection.

    -Please inform us of any chronic medical condition that could affect your child while at school.  There are care plans for specific conditions located in the "Health Forms" section of this web page.  Any others can be addressed by email.

     -The clinic does not stock medications.  All medications (over-the-counter and prescription) for student use must be brought to school by a parent or guardian in the original container. If submitted for "as needed" use, these must be picked up before the last day of the school year.  No "once daily" or "twice daily" medications are given at school unless directed by a physician in writing.  Please see Forsyth County's "Request for Administration of Medications" form in the "Health Forms" section of this web page.

    -If your child is sick, running a fever, or has tested positive with COVID 19, please DO NOT send them to school.

    Services provided by school nurses include:

     

    1. Promotion of healthy lifestyles.

    2. Health care counseling or guidance to students, their family, and to staff.

    3. Identify health concerns by observation and assessment of needs, and provide appropriate care.

    4. Medication management

    5. Immunization verification according to state guideline requirements.

    6. Screenings: vision & hearing

    • Band-Aids-all sizes
    • Zip baggies-sandwich size
    • 3 ounce cups
    • 4 x 4 gauze
    • Anti-Bacterial Soap
    • Tissues (Kleenex)
    • Feminine products
    • Lysol Spray
    • Gloves
    • Graham crackers
    • Saltine crackers
    • Coke or Pepsi
