In order to ensure that all notices of transportation changes include all necessary information, we have created a form for you to fill out.  Every time your child will go home in a different mode than usual, we ask that you print it, complete it entirely and send in with your child to give to his/her teacher.  Your child's safety is our priority and we are dedicated to making sure that all students get home correctly.
