Below you will find the registration form for the 2023-2024 school year. Please read the following instructions carefully!
Please open the attachment, read the information, download and fill out the registration form, save it as a PDF. Please make sure to fill out the form completely or it will be returned. We will NOT accept handwritten applications. Please email the form back to Linda Bell (lbell@forsyth.k12.ga.us) as an attachment. We are NOT accepting payment for the registration fee at this time. You will get an invoice when your registration form has been accepted . Please be aware you are not guaranteed to be in the program until our form and payment have been received.
