Rising 11th Grade TDAP & Meningitis Information

Attention Rising 11th Grade Students:

Dear Parent/Guardian, 

According to Georgia State Law, children 16 years of age and older, who are entering 11th grade must receive one booster dose of the meningococcal conjugate vaccine (MCV4), unless their initial dose was received on or after their 16th birthday.  Please note that your child MUST be 16 years of age to receive this booster dose.  

ºìÐÓÖ±²¥app must receive an updated immunization form (GA 3231) for your child before the 2024-25 school year starts.   

If you wish to seek an exemption, you must provide one of the following:  

·       GA Form 2208 (Affidavit of Religious Objection to Immunization)  

·       Medical exemption noted on GA Form 3231 by the physician.  

Meningococcal disease is a serious bacterial illness that affects the brain and the spinal cord which can cause shock, coma, and death within hours of the first symptom.  Please contact your child’s healthcare provider or the Forsyth County Health Department located at 428 Canton Rd, Cumming, GA  30040 (770-781-6900) at your earliest convenience. If you need any assistance or have any questions, please contact your child’s school nurse. Once your child has received their MCV4 immunization booster dose, please provide a copy of their updated GA 3231 form to your child’s school nurse.
